+ Position Purpose: The Home Health and Hospice Registered Nurse is responsible for providing nursing care for patients in the home health and hospice programs. Essential Functions and Basic Duties: Primary Duties o Under the direction of the patient's physician, develops the plan of treatment, makes necessary revisions, then initiates, evaluates and follows the treatment plan. o Makes initial assessment visits and conferences with attending physician to develop the nursing care plan for the patient. o Initiates OASIS documentation in EMR and submits to supervisor for review. o Initiates appropriate preventative and rehabilitative nursing procedures as ordered by the physician to provide total patient care. o Demonstrates and teaches health care to patient and/or caregivers to promote independence. o Assists families in recognition and solution of physical, emotional and environmental health problems and accepting responsibility for care. o Assists patients as needed to transfer, lift, turn, assist in ambulation, active/passive range of motion, and other physical functions needed in the performance of duties while in the patient's home. o Participates in on-call assignments. o Active Participation in the quality program. o Participates in integration of nursing services in an overall comprehensive care plan for the patient. o Determines priorities of patient service based on patient/caregiver needs and resources, using professional judgment. o Observes, evaluates and reports to physician the patient's physical and emotional condition, reaction to drugs or treatment, interpret social and physical factors in the environment that effect patient care. o Evaluates the effectiveness of nursing service to individuals and families thought case conferences with interdisciplinary team. o Attends and participates in staff meetings, professional conferences, in-services, and continuing education programs. o Provides information to agency regarding changes in status of patient. o Maintains necessary records and forms for analysis and planning of service for establishment of priorities for care. o Records all nursing service, communication and conference, utilizing appropriate forms, consistent with agency record policies. o Performs all record requirements in a timely manner. o Assigns staff based on specific patient needs, family capabilities, staff training and experience and the degree of supervision needed. o Performs home health aide supervision every two weeks or any time there is a questions of change in the patient's condition. Is readily available for consultation to aides on the patient's status. o Assists in coordinating all services provided o Provides written instructions and supervisory visits for home health aides to ensure adherence to the plan of treatment o Provides written assignments/instructions for other staff involved o Supervises and consults with the Licensed Practical Nurse as necessary. o Supervises and consults with the Certified Nursing Assistant as necessary, minimally documenting C.N.A evaluation for each case assignment every 14 days. o Performs other duties as assigned. Secondary Duties o Demonstrates 100% commitment to performing according to the CHOICE values of TMH and representing the organization in a positive and professional manner. o Establishes and maintains effective communication and good working relationships with all co?workers. o Adheres to TMH and department dress code policies and procedures. o Utilizes initiative; strives to maintain steady level of productivity; self-motivated; manages activity and time. o Completes annual education, training, in-service, and certification requirements; attends departmental staff meetings or reads meeting minutes. o Maintains regulatory requirements. o Maintains patient confidentiality at all times. o Reports to work on time and as scheduled; completes work within designated times. o Actively participates in departmental and facility performance improvement and continuous quality improvement activities. o Strives to ensure continual compliance with departmental, hospital, state and federal regulations and policies. o Follows policies and procedures for infection control, safety, and risk management to ensure a safe environment for patients, public and staff. Qualifications: Education/Certifications o High School Diploma or GED o Current, Valid Driver's License o Current, Valid Colorado Registered Nurse License Knowledge Required: Experience Required: o Minimum 1 year experience in an acute care setting o 1 year experience in home health or hospice preferred Skills/Abilities: o Demonstrated ability to work independently, make accurate, and at times, quick judgments o Ability to respond to crisis outside of a hospital setting. o Must demonstrate acceptance of and adaptability to different social, racial, cultural and religious modes. o Must be able to cope with mental and emotional stress and demonstrate emotional stability. o Demonstrated ability to read, write, and carry out written and verbal instructions in English o Must possess good verbal and written communication skills o Must have transportation available to make home visits. Work Environment: This position includes work in both an office setting and in patient's homes. Be able tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials. Work indoors in patient homes and in-patient hospice environment. Travel to and from patient homes. Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
